If paying by P.O. or INVOICE - Send completed Registration Form and a copy of P.O. either by MAIL or FAX (see contact information above). Organization's FEIN (Federal Employer Identification Number) is required when paying by P.O. or Invoice. Please include FEIN when submitting registration materials.

Cancellation/Refund Policy - Cancellations must be submitted in writing by January 23, 2020 in order to recieve 50% refund. No refunds will be processed after this date. Please email outreachregistration@niu.edu or fax to (815) 753-6900.

Speaker Handouts - Handouts will only be available via ISHA's Website (www.ishail.org).There will be no paper speaker handouts distributed at the convention. All handouts provided by both invited and submitted speakers will be available starting in January on the ISHA website.
